• You will be asked to confirm your appointment, via telephone or text message, and complete a COVID-19 screening questionnaire. *We regret that we are unable to hold unconfirmed or unscreened appointments*
  

• Please reschedule if are experiencing any flu-like symptoms (cough, fever, etc.), have had contact with someone who has tested positive for COVID-19, or have traveled within two weeks of your appointment.
  

• Please wear a mask/face covering to your appointment
  
• Please sanitize you’re hands upon entry
  
• Please do not bring anyone to your appointment who is not absolutely necessary
  
• Please wait in your car and call us once you arrive. You will be instructed when to enter the office
  
• Please leave any unnecessary belongings in your car
  
• Your temperature will be taken with a forehead thermometer upon entry. If you have a fever, your appointment will be rescheduled
